The Following Items are Forms and Material List for your Choral Student. 

Please Fill out the following forms and return them to the choir room by the specified date. 

__Choral Materials-Due Friday,  Feb. 1^st

__Outfit Form(all outfit forms)- Due with payment-Friday, Feb. 1st

____Parent Volunteer Form- Due Friday, Feb. 1st

____Choral Contract- due by Friday, Feb. 1^st

___Choral Boosters- due by Friday, Feb. 1^st

___Medical Form- Due by Friday, Feb. 1^st

__Choir Letter Form- (For Student to Fill Out)

___Reason of Absence Form-(For you to keep in your records)

___Choir Service Form

___T-shirt form –due by Feb. 1^st ( We must have at least 12 t-shirts of each item ordered in order to purchase t-shirts.)

 

 

Materials: (Due Friday)

#2 pencil

Notebook-3 ring view binder at least 1 ½ inch to 2 inches.

Dividers- with tabs

Box of tissues-to be turned into the choral director (optional but greatly needed)

Bottle of Hand Sanitizer – to be turned into the choral director (optional)

Show Choir- basket or box to keep items inside your cubby or on your shelf
